法国波尔多大学FX Mahon教授访谈

　　《肿瘤瞭望》：对于慢性髓系白血病（CML）患者，我们什么时候考虑停止或者继续TKI治疗？

　　Mahon教授：我们在今天的会议上讨论了是否停止治疗。当患者获得深度分子学反应我们可以停药，但前提是持续的分子学反应，意味着患者获得深度的治疗反应需要持续一段时间。我们目前对持续治疗反应的定义为2年，因此当患者获得深度分子学反应（BCR-ABL1融合基因定量下降4.5 log水平）后，在考虑是否停药时，你需要等两年的时间。

　　We have a debate today on stopping or not stopping treatment. You can stop treatment when the patient has reached a deep molecular response but it is necessary to achieve a sustained deep molecular response， which means that deep response needs to continue for a period of time. Our definition of a sustained response is two years， so when a patient achieves a deep molecular response (a 4.5-log reduction in BCR-ABL1) you should wait two years before considering stopping treatment.

　　《肿瘤瞭望》：临床上总有一些患者不能获得深入的分子学反应。您认为干细胞移植和免疫调节治疗哪个对CML患者更有效？

　　Mahon教授：目前还很难说，这主要和白血病细胞本身的生物学特征相关，而不是考虑免疫学上的控制，因为暂时还没有免疫控制这方面的证据。

　　It is difficult to know. That is most likely linked to the biology of the leukemic stem cells themselves rather than the immunological control because we have no evidence of immunological control.

　　《肿瘤瞭望》：在避免CML患者复发过程中，免疫调节治疗发挥非常重要的作用。在停止治疗后，免疫标记物（细胞因子或者免疫细胞亚型）的检测是否必要？

　　Mahon教授：不需要。目前我们唯一知道的就是自然杀伤细胞，但是我们并不知道如何调节这类细胞。另一个比较推荐的方法就是使用PD-1拮抗剂和PD-L1类药物，它们可以用来清楚白血病干细胞，但是目前主要问题是这些药物的毒性反应。

　　No. The only thing we know of is natural killer cells， but we don’t know how to modulate these cells. Another encouraging technique is the use of the anti-PD-1 and PD-L1 drugs which could be used to eliminate the stem cells but the main problem is the toxicity of these drugs at the moment.

　　《肿瘤瞭望》：对于获得完全缓解并停止TKI治疗的患者，我们应该如何临床检测这些患者，时间间隔多长？

　　Mahon教授：至于随访，我们视缓解情况需要每3~4个月对患者进行PCR定量检测。最好使用国际标准进行PCR检测，好的实验室十分重要。

　　To follow patients， you need to perform quantitative PCR every three or four months depending on the remission. It is important to have a good laboratory which performs PCR using the International Scale.
